Which appointment should I make?

If it is your first appointment or you are treating a new condition, please select "Initial Evaluation". This appointment will consist of tests and measures to accurately diagnose your physical therapy condition. You WILL begin treatment interventions to start your road to success!

If you are a returning client and are not treating a new condition, please select "Treatment".

If you are looking to enhance your health and wellness, improve your craft and/or prevent injury - please select "Wellness and Performance"

Do I need a referral?

It is not required to have a referral to seek physical therapy services, though insurances have unique processes. Please check with your insurance company, if you chose to use one. If you do have a referral or imaging/protocols - please have this for your first appointment.

Do you take insurance?

At this time, we accept Blue Cross Blue Shield of MN or cash for service. We accept cash, check, debit or credit card (FSA/HSA is acceptable).

Can I request reimbursement if out of network?

Yes, If you chose to submit a claim to your out of network insurance. Upon request, you may be provided with a superbill and ICD10 codes in order to submit your claim. Please check your insurance provider information/account for the process required.

How much does a visit cost?

Phone consult: Free

Initial Evaluation: $180

Treatment Session: $120

Wellness and Performance: $120

What should I do to prepare for my 1st visit?

Please wear comfortable clothing that you can move in. If you have pets, please ensure they are not able to access the area of your home treatment will take place. Arrange your home to optimally serve you in performing mobility - it is okay if your space is small! Your physical therapist will come equipped with the essentials for examination and treatment: this may include a treatment table, theraband, handweights, manual therapy supplies. If your home is not a safe environment for treatment, we reserve the right to deny or discontinue services.

Do you come to my house? Or do I come to a clinic?

Mobile Body/Mobile Tots LLC does not have a clinic location, allowing you to receive treatment in your home or gym.
